Grace365 Reentry is a transitional living facility for women over the age of 18 who are leaving correctional facilities or homelessness and looking for an environment where they can grow and change spiritually and personally. The transitional living facility is only able to house a limited number of residents at a time, however, and so the program must make decisions on who will be admitted

Admission

The following outlines the admissions process

1. Initial Inquiry and Pre-Screening:

Pre-Screening: We will conduct an initial pre-screening to determine an applicant’s eligibility, which may include assessing their current housing situation, identifying any specific needs, and explaining the program’s requirements and expectations.

2. Application Submission:

  1. We will process application 3-5 business days. However, depending on emergency situations, application can be processed within 24 hours. 
  2. Only applications by women over the age of 18.
    1. Applicants must also display a strong desire for spiritual and personal change. 
  3. If a prospective resident for some reason cannot access the digital application, or is unable to complete the application and assessment by themselves due to a disability, Grace365 Reentry will provide alternate routes of application so that all applicants can receive equal consideration.
    1. Grace365 Reentry staff reserve the right to use their discretion when approving residents.
  4. Once accepted, applicants will receive welcome email or phone call.

3. Interview and Assessment:

Interview: Conduct an in-person or virtual interview to better understand the applicant’s background, situation, and goals.

Assessment: We evaluate the applicant’s suitability for the program, taking into account their willingness to adhere to program guidelines, participate in support services, and actively work towards their transition to stable housing.

4. Orientation and Agreement:

We provide a comprehensive orientation to the program, including an overview of rules, expectations, available support services, and the transitional housing contract.

Contract Agreement:** Have the applicant sign a formal agreement outlining their responsibilities, the program’s guidelines, and the expected duration of their stay.

5. Intake and Move-In:

Intake Meeting: Conduct a formal intake meeting to finalize the admission process, review program guidelines, and assess any immediate needs (e.g., clothing, toiletries, bedding).

Move-In: Assist the applicant in moving into their assigned housing unit and provide a welcoming and supportive atmosphere.

6. Case Management and Support Services:

Initial Assessment: We develop an initial case plan for each resident, identifying their goals and the support services they will receive during their stay.

7. Ongoing Monitoring:

Regular Reviews: Conduct periodic reviews of residents’ progress, adherence to program rules, and changes in their circumstances.

Graduation Planning: Collaborate with residents to create a plan for their transition to stable housing or other appropriate living arrangements.

8. Program Exit:

Graduation: Celebrate residents’ successful completion of the program as they transition to more stable housing.

Follow-Up Support: Offer continued support and resources to help graduates maintain their independence and housing stability.

Items to Bring

  • Five (2) sets of clothing, personal hygiene items, no perfume, no mouth wash, or any products containing alcohol.
  • Bring: wash cloths, towel, nice clothes for church, shower shoes and robe.
